Tax receipt information form
Income tax receipts for donations can be claimed by either spouse or common-law partner in accordance with the Canada Revenue Agency’s guidelines. As such, donations made via tuition payments are issued in the name of both parents at the mailing address listed in the student’s official record with Marianopolis College. If you require a change to this information for tax purposes, you must fill out the form below.
Updating your information via this form only extends to the issuance of tax receipts by The Marianopolis Millennium Foundation and does not alter the information in the student’s official Marianopolis record. Students should report any change in their situation (e.g. recent move, parental separation or death) by updating their record in Omnivox.
By filling out this form, you attest to the accuracy of the information which The Marianopolis Millennium Foundation will then use in good faith to issue your receipt. For information on a charity’s obligations concerning issuing a receipt in a name other than the donor’s, please visit the Canada Revenue Agency’s website.
Income tax receipts for tuition fees paid in 2014 (T2202A and Relevé 8) are available online via students’ Marianopolis Omnivox account. Former students who have forgotten their access information should contact the Records Office, 514.931.8792, ext.300.
