Indicate below all the availability you have to receive peer tutoring.  A minimum of 3 one hour time blocks, outside of activity period, is required in order to complete this form.  Students who qualify for peer tutoring will receive 1 hour of tutoring a week in the Learning Resources Centre.

Why do you think you need peer tutoring?

 

PLEASE READ CAREFULLY BEFORE SUBMITTING

By registering in the Peer Tutoring Service, I agree to the following obligations:

  • I will receive free tutoring services from the tutor assigned to me by the Learning Resources Centre (LRC).
  • I understand that all tutoring sessions must take place in the LRC (room F-317) at regularly scheduled appointment times.
  • I understand that I must show up on time or advise my tutor at least 24 hours in advance that I cannot attend the predetermined appointment. 
  • I will attend all my tutoring sessions unless I have a very good reason to cancel. Not showing up or asking to reschedule more than once may lead to the cancellation of my privilege.
  • I understand that not attending my classes regularly will result in the termination of further tutoring assistance.
  • I understand that the tutor will do her/his best to help me but should not be seen as a replacement for my teacher.
  • I will try to benefit as much as possible from this service by coming prepared to the sessions with the appropriate material and questions/topics to be discussed.
  • I will not ask the tutor to do assignments or homework for me.
  • I will promptly advise the LRC of any difficulty associated with my tutoring sessions, particularly if I no longer require the service.
  • I will be receiving tutoring for no more than two disciplines at a time.
  • I, freely and voluntarily, accept to complete an evaluation questionnaire at the end of the semester.

By clicking on the submit button below, I understand the above obligations and will abide by them if granted the service.  The application form will be sent to the Learning Resources Centre and I understand that the allocation of spaces for the service is made on a priority basis and is subject to the availability of tutors.  Selected applicants are contacted via MIO (Omnivox).
